I certify that all the information set forth in this Employment Application is true and complete. I understand that any omission from, or any false or misleading statements on this application may result in rejection of my application for employment, and if I am employed, termination of employment. I understand and agree that Executive Hotel Management, Inc. or EHM-LA, Inc. (such relevant company or companies, the “Employer’) may require me to submit to a test for the presence of controlled substances in my system prior to my employment and at any time during my employment, to the extent allowable by law. I also understand that any offer of employment may be contingent upon, among other things, the passage of a physical examination. I consent to disclosure of the results of any physical examination and other employment related tests to the Employer. I further understand that the Employer may require me to submit to a background investigation prior to any employment with the Employer and at any time during employment with the Employer. I understand that such background investigations, which may be done by the Employer or a through an investigative agency or bureau of their choice, may include but not be limited to criminal convictions, any tests to determine illicit drug use, company property theft and company rules adherence. I release and waive any claim I may have against people and organizations as a result of their good faith compliance with information requests and/or against Employer and its officers, directors, employees and other representatives for seeking, gathering and using such information for employment purposes. I understand and agree that if I decline to consent to background investigations required by the Employer, in its sole discretion and to the extent authorized by law, or decline to sign this Consent or decline to agree to take any of the above tests, my application for employment may be rejected or my employment may be terminated. I understand and agree that if employment is offered to me and I accept, my employment may be terminated at any time, with or without cause and with or without notice, by myself or the Employer and that no employee or officer has the authority to offer or promise me employment for any specified period of time and further that no employee or officer has the authority to offer or promise me raises or promotions unless such promotion or raise shall be in writing signed by Evan Rayman. I understand that any employment will be governed by the policies and procedures of Employer in effect from time to time and I agree to comply with such policies and procedures.
